Client Assessment & Care Planning

All of our care plans are firmly based on the clients’ needs, whilst balancing the views of the family and carers. We follow therapeutic evidence to ensure our services are up-to-date and evidence based. A rigorous assessment is undertaken for all our clients, which looks at all their needs ranging from moving and handling to the spiritual or emotional, and including the needs for stimulation and fun. We also feel that it is important for our clients to be involved in planning their own care.

Our assessments are all based on an initial community risk screen, which highlights risks broadly, and leads to a subsequent series of individualised risk assessments to ensure that risks are managed effectively. Care plans are then developed to meet the needs identified within the risk assessment process, and these care plans will have input from other members of the multi-disciplined team as appropriate.
